If you using IE7 then try this:

In Internet Explorer Menu bar, go to: Tools, Internet Options
Now click on Content tab
Now click on Settings button for AutoComplete
Now you will have various options to decide. In your case you
definitely need to Tick on: "Usernames and passwords on forms" BUT also
tick on "Prompt me to save passwords"

The procedure was same in IE6.

Thank you for your timely response. I am running IE6. The items you
referenced were already set to this. Do you have any other ideas?

Hi,
First:
Use IE PassView (NirSoft)
http://www.nirsoft.net/utils/internet_explorer_password.html to clear any
autocomplete data for that site.

If that didn't reset the prompt for that site, use IE Passview to recover or
export autocomplete data, then clear all stored login data from Internet
Options> Content> Autocomplete. This should reset the prompt for all sites.

Hope this helps,
Don
